2 Scope of disclosure, etc.
The scope of disclosure, etc. shall include the name, address, and purpose of use of the person, as well as personal data in our possession that we have obtained and are currently holding. 3.

For certificates, etc., only the part that contains the name, date of birth, and address of the applicant can be used for identification. In the case of a personal identification number card, please photocopy only the front of the card, and in the case of a certificate that includes your permanent address, basic pension number, etc., please cover the permanent address, etc., with a piece of paper, etc.
If your current address is different from the address we have on file due to a change of residence, please submit a certificate of residence or a copy of your family register that shows the history of your change of residence.
If we are unable to verify your identity because the required documents are not enclosed or the documents are unclear, we will return the set of documents you submitted. We apologize for the inconvenience, but please prepare the necessary documents and submit the request again. 4.

If the request is made by a representative (person with parental authority, guardian of a minor, conservator, assistant, guardian of an adult, voluntary guardian, or representative authorized by the person), please submit all four of the following documents.
